Reporting requirements: Please include all materials of biological origin, including toxins, that are used or stored in your labs or other areas under control. Simply place a check each box beside each category of biological material that is present in one of your labs. If no materials of biological origin are used or stored in any or your areas, then please check the appropriate box.
How to Fill Out This Form: All materials of biological material should be fully described in Section 2 of this form, and any recombinant materials should be fully described in Section 3.
Guidance for Section 2 of this form: Please describe the biological material as fully as possible. Be sure to include genus and species where appropriate. If you have multiple strains of materials used or stored in your labs or other areas under control, you DO NOT need to list all strains of the material if they are of equal pathogenicity. For example, multiple strains of E. coli K 12 do not need to be individually listed, just indicate E. coli. When describing cell lines, please include tissue source and type (e.g., human HELA cells, Human embryonic kidney cells), primary cell lines should be identified as such. Viable: Please use the drop-down box to indicate whether the biological material described is viable or not. Pathogen: If the biological material is a human, animal, or plant pathogen, please use the drop down box to indicate such. Drug Resistant: If the biological material is drug resistant, please use the drop down box to indicate such.
Locations(s) Where Used or Stored: List the building and room number where the biological is used or stored. Where appropriate specific storage locations may also be indicated (i.e. -80 freezer).
